	Loss Prevention Survey Reports This can be downloaded, completed electronically and emailed in a typed Microsoft Word format to British Marine only no later than 24hours after the survey has been completed. Note however that a copy must be signed for receipt and included with the report.
	Risk Analysis Summary (Example) This shows a fictitious completed form as a worked example.
	Ultrasonic Leak Test of Hatch Covers This report form should be used where ultrasonic testing of the hatch covers is carried out.
